Privacy Notice

Your information is collected securely over the internet with 256-bit SSL encryption security.

We NEVER sell or rent your contact information, EVER.

We help clients determine their coverage needs and offer to help clients and prospects determine if their providers and pharmacies are in-network and if and how their medications are covered under their plan.  We do not use any personal or protected health information obtained through this process for any other purpose, except where health questions are required to enroll in a particular plan (e.g., a chronic special needs plan, short-term health insurance, and other coverages that include asking health questions as a condition for obtaining coverage or for determining premium class).

We utilize electronic application systems such as Sunfire, Connecture, Ascend, Health Sherpa, Sunshine Life and Health, America’s Choice,  online carrier systems, and paper application forms to submit enrollments for coverage and ALWAYS obtain consumer consent through electronic signatures, fax, or wet signatures before submitting these applications.

We also systems such are RetireFlo and Search and Save to maintain client and prospect medication and provider information.

We follow HIPAA, HITECH, CMS (Centers for Medicare and Medicare Services), state regulatory and insurance company guidelines and requirements. We also follow Affordable Care Act, marketplace and IRS privacy and security guidelines and requirements and are annual signatories to both the Individual Marketplace General, and Privacy and Security Agreements.  We also have an Agency Privacy and Security policy and have completed the New York State-required cybersecurity template.

We send and receive email replies containing Protected Health Information (PHI) and Personally Identifiable Information (PII) using the PauBox secure email system using pcholak@getinsuranceanywhere.com as the email address. The security of these emails can be verified by determining that the bottom of the email contains this message: Secured by Paubox - HITRUST CSF Certified. The Paubox system does not require the user to use a password to open the emails or to reply to those emails. If the email asks the receiver to forward any protected information, or if the sender wants to reply on a secured, encrypted basis, s/he must click on REPLY and forward the email back to the sending email address. If the person replying attempts to send a separate email from his or her email system, that email will not be encrypted unless that person has encryption software on his computer.

When forwarding PII and/or PHI by fax, we use a fax cover sheet and fax only to secure and attended fax machines. Our fax machine is secure and can be accessed and used only by the principal.

We follow the “minimum necessary” rule in forwarding and collecting PII and PHI.

Before or at the time of collecting personal information, we will identify the purposes for which information is being collected.

We collect and use personal information solely with the objective of fulfilling those purposes specified by us and for other compatible purposes, unless we obtain the consent of the individual concerned or as required by law.

We only retain personal information as long as necessary for the fulfillment of those purposes.

We collect personal information by lawful and fair means and, where appropriate, with the knowledge or consent of the individual concerned.

Personal data must be relevant to the purposes for which it is to be used, and, to the extent necessary for those purposes, should be accurate, complete, and up to date.

We will make readily available to customers information about our policies and practices relating to the management of personal information.

Unless you ask us not to, we may contact you via email or phone to tell you about new products or services and/or to explain the features and benefits of any of our programs. If you provide your email address, you also give us permission to send you a monthly newsletter about health and/or life, disability, or long-term care and short-term (recovery care) insurance.

We protect the confidentiality and security of your personal and health information. We maintain physical, electronic, and procedural safeguards to protect your personal and health information.

Affordable Care Act Consent Form. We obtain consumer consent (either via a recorded telephone call, text message, email, electronic document with digital signatures, or physical document with wet signatures prior to accessing or updating their information on healthcare.gov when helping them enroll in any plan offered on healthcare.gov.  Through such consent the consumer authorizes us only to view and use the confidential information they provide only for the purpose of one or more of the following:

  1. Searching for an existing Marketplace application;
  2. Completing an application for eligibility and enrollment in a Marketplace Qualified Health Plan or other government insurance programs, such as Medicaid and CHIP or advance tax credits to help pay for Marketplace premiums;
  3. Providing ongoing account maintenance and enrollment assistance, as necessary; or
  4. Responding to inquiries from the Marketplace regarding the individual’s Marketplace application.

The consent also contains a provision that will we not use or share any personally identifiable information (PII) for anything other than the above purposes and that we will ensure the consumer’s PII is kept safe when collecting, storing, and using PII for the above purposes.

We also obtain the consumer’s confirmation that the information provided for entry on the Marketplace eligibility and enrollment application is true to the best of the consumer’s knowledge.  The consumer confirms his understanding that he does not have to share additional personal information about his health with us beyond what is required on the application for eligibility and enrollment purposes.  The consumer agrees that consent remains in effect until he or she revokes it and that the consumer may modify consent by calling Paul M. We collect Personally Identifiable (PII) and Personal Health Information (PHI) only to the extent necessary to determine your eligibility for or to help you select insurance coverage; to complete or help complete your application; help correct billing or other errors with your insurance company; or to explain your policy's coverage and network provider provisions. If you request an Affordable Care Act quote on this website, the quote form will not progress unless and until you provide the required information. Unless you provide this information we will be unable to provide you with a quote.

Applying for Insurance. If you create an account on one of the exchanges and ask us to assist you in making application and/or choosing a plan; ask us to help you apply for an Affordable Care Act plan involving the use of web broker software; or apply through a carrier's website, use a paper application, or by telephone, we (or the carrier) may ask you to provide personal, financial, and/or health information about you or one of your dependents. This information helps determine plan eligibility (and is also used to verify whether your provider accepts the plan and if your medications are covered under the plan if you are applying for a health plan). We follow CMS, marketplace, IRS, HIPAA, and HITECH rules in collecting this information. If you apply through us for government health insurance subsidies or for health insurance sold through a government-run exchange, we will ask you for personal information, including financial, tax filing and household information, that we will enter into sites and/or forward to entities responsible for processing such subsidies and applications.

If you apply for another type of policy (e.g., life, final expense, Short-Term Health insurance and/or supplemental health insurance that requires any form of full, expedited, or simplified underwriting), we will request PII and PHI that the carrier requires in order to review and process your application. We cannot help you apply for one of these types of policies unless you provide this information because the carrier will not process your application unless they receive it.

Security of information. We send information containing personally identifiable information (PII) and/or protected health information (PHI) by secure encrypted email; utilize secure government, web brokerage, or carrier websites; or fax the information (using a coversheet) to an attended secure fax. Our computers are all encrypted. We take precautions to protect your information both online and offline. We are prohibited from asking you for your password to your healthcare.gov account and cannot make an application for you by using your healthcare.gov account. However, with your permission, we may guide you through the process of making your own application through healthcare.gov.

Wherever we collect sensitive information (such as income or information about health conditions for underwritten policies including life or final expense insurance; Short-Term Health insurance; or a supplemental health insurance policy), that information is encrypted and transmitted to us in a secure way. You can verify this by looking for a closed lock icon at the bottom of your browser or looking for "https" at the beginning of the address of the web page.

While we use encryption to protect sensitive information transmitted online, we also protect your information offline. Only those who need the information to perform a specific job are granted access to PII or PHI. The computers in which we store PII and PHI are kept in a secure environment.

Providing you with a quote or processing your application. We may use your personal information to get in touch with you when necessary to process your application or to provide you with a quote. For example, we may call or email you throughout the application process to inform you of the status of your application and to seek additional information that is requested as part of the application.

Privacy and security policy. We have developed and implemented a privacy and security policy and have completed a model cybersecurity template provided by New York State,

Cookies. This website uses cookies (small simple and benign text files temporarily stored on your computer) for the sole purpose of saving your information for you as you fill out our Affordable Care Act quote form. This is for your convenience so that your browser will remember your form entries if you decide to go to a previous page to check a previous answer. Once the quote form has been completed, the cookie is deleted from your computer by your browser, and our system deletes our copy of the cookie (called a "session").

The information gathered by the cookie is whatever information you put in the fields of the quote form. Once you submit the form the cookie is no longer needed, so it is deleted.

Your experience on our site other than for the Affordable Care Act Quote form will not be affected by whether or not you accept cookies from this site. You must accept cookies in order to fill out the Affordable Care Act quote form.

2025 Annual Enrollment Period For Medicare Beneficiaries

The Annual Enrollment Period (AEP) for enrolling in plans effective January 1, 2025 has ended. The AEP runs from October 15 to December 7 annually.

Enrollment in a Medicare Advantage, Medicare Advantage Prescription Drug, or stand-alone Part D Drug plan can now occur ONLY if a Medicare beneficiary is eligible for another election period [e.g., the Individual/Individual Coverage Election Period (ICP or ICEP)] when first becoming eligible for Medicare; a Special Election Period (for those who experience qualifying life events like an involuntary termination of their existing plan, moving outside of the plan’s service area, losing or becoming entitled to Medicare or Extra Help, declaration of a weather related emergency, etc.), or the Open Enrollment Period. Except for individuals desiring to enroll in a Chronic Special Needs Plan, there are no health questions to qualify.

Medicare beneficiaries can enroll in a Medicare Supplement plan within 6 months of their Part A and B effective dates without answering health questions. Generally, individuals with Medicare Supplement plans can change plans at any time but in many cases will need to answer health questions to qualify. Individuals with Medicare Advantage plans can enroll in Medicare Supplement plans during the Annual Enrollment Period or Open Enrollment Periods but in most cases will have to answer health questions. There are special rules for individuals with “trial rights” or eligibility for guaranteed issue policies that don’t require answering health questions.
